Stonegates Health Center provides a traditional arrangement of one and two room suites. The Center includes a private dining room, where appetizing meals are served, a living room, where there are a range of social activities available and a smaller lounge with a fireplace for small gatherings and libations. Stonegates offers 49 medicare certified beds for our residents.
Other services include: Our premier 24-hour Skilled Nursing Care and rehabilitation services, which include Occupational Therapy, Physical Therapy and Speech Language Pathology. All other health care needs can be met within our Health Center from our ancillary partners that includes pharmacy services and diagnostic services.
Plans have been approved and construction has begun for our new Stand-Alone Assisted Living and Memory Care apartments. Stonegates provides a continuum of care with Assisted Living options as well as Memory Care options that are customized to meet each resident’s needs.
Stonegates offers for our residents the opportunity to enroll in our Recreational Club which provides necessary care for loved ones while they immerse in a fun engaging ad stimulating environment. Our Certified Nursing assistants and licensed Nurses will provide residents with personalized care, assistance, and supervision. This program offers support to family members and caregivers, allowing them to handle personal business, or just relax, knowing that their relative is well cared for and safe.

The Health Center
Stonegates Health Center offers exceptional skilled nursing services in a beautiful home-like setting. Our 49-bed, Medicare-certified Health Center offers primary care nursing when you need it.
Residents in our community have access to our 24-hour nurse call system that alerts the Health Center should a medical emergency arise. Residents can also maintain their privacy and dignity by taking advantage of our Resident Support Services when necessary. These services are tailored to give assistance to residents, when needed, in their own home. This continuum of care allows our residents to live independently as long as possible in an elegant community setting.
Healthcare at Stonegates goes well beyond the typical skilled nursing facility. With a very high nurse to patient ratio, all patients receive hands on care from a skilled nurse every day. We strive for consistent assignments so that residents get to know and build relationships with the staff. Our goal is to combine caring relationships with a high level of professional nursing care.
Structured activities provide socialization in a home like setting. In addition, our Health Center offers customized care in a soothing, positive environment for people with Alzheimer’s disease and related memory impaired disorders.

In a traditional assisted living center, services are provided in designated residences within the retirement community. In contrast, Stonegates provides a continuum of assisted living options that are customized to meet the needs of each resident. We believe that our residents should be able to access additional care support without leaving their home.
Often, residents need additional services but prefer to remain in their retirement units. At Stonegates, residents in cottages and apartments can arrange for additional companion care, which may help a resident retain their independence. Stonegates staff can deliver dinner trays daily, if needed, increase housekeeping services and have a driver available for errands or other transportation needs.
Our Resident Support Services (RSS) also provides a registered nurse who can arrange for a medication delivery system, Simple Meds. With this system, medications are packaged by day and the time of day the medications are to be taken in easy tearoff packets. The RSS nurse will provide medication management, which includes handling refills, changes in medications and oversight to assure the medications are being taken safely.
For those residents who would like to move to a more traditional arrangement, we have one and two room suites in our Health Center. This center includes a private dining room, and a lounge with a fireplace with cocktails served in the evenings. We offer a range of social gatherings as well as trips to local restaurants and other areas of interest. Residents who choose these options would also avoid another move if their health care needs would increase.
